Aging and Disability Services (ADS) is the Area Agency on Aging for Seattle-King County. We plan, coordinate, and advocate for a comprehensive service delivery system for older adults, family caregivers, and people with disabilities throughout King County.
ADS is committed to reducing unnecessary readmissions at local hospitals. We collaborate with health care providers to enable more effective transitions from hospital to home. We coach patients and their caregivers to follow their physicians%u2019 discharge orders and manage their health care more effectively.
