WHO SHOULD ATTEND? Provider agencies that support individuals with behavioral healthcare needs that are living in (or at risk of) Adult Care Homes and may be interested in transitioning to their own homes in the community. This includes providers of enhanced services such as Assertive Community Treatment Teams, Community Support Teams, as well as providers of (b)(3) services Peer Support, Individual Support, and Supported Employment (IPS-SE). Please plan to attend the training that is designated for the primary region where you provide services.
TRAINING TOPICS - Learn about the settlement agreement between the US Department of Justice and the State of North Carolina - Learn expectations of the provider's role before, during and after the transition process - Learn real, practical, "how to" information including step-by-step instructions - Network with providers who have assisted with transitions - Hear about the successes and the challenges of individuals who have transitioned to the community - Learn the roles of Cardinal Innovations' Transitions to Community Living Staff - Learn about providing services to individuals who chose to continue living in Adult Care Homes
