Questionnaire Review
Some limitations to keep in mind:
1. "no answer" is a default setting for radio button responses and always appears first
2. there is currently no option to make multiple radio button selections. With the order they're currently in, largest audience to small, this should be good enough data.
3. no option exists to limit the zip code to 5 digits
4. BPT said some clients include a SurveyMonkey link in their confirmation messaging, but they typically see a very low completion rate if it's not part of the checkout process, optional or not.
5. Note that questionnaires only display on desktop view, but not on mobile.
