The NC-SNAP is a needs assessment tool that, when administered properly, measures an individual's level of intensity of need for developmental disabilities (DD) supports and services. NC-SNAP certification is available to professional DD staff who are responsible for completing or reviewing NC-SNAP assessments as part of their job responsibilities. Cardinal Innovations has arranged with the state to offer additional NC-SNAP training in Chapel Hill(September 14) and Charlotte (September 25).
If providers cannot attend one of these training dates/locations, please arrange to attend one of the monthly training sessions offered at Dorothea Dix Hospital in Raleigh. See schedule at the attached link. www.ncdhhs.gov/providers/provider-info/mental-health/nc-support-needs-assessment-profile
