IDD Provider Training: Transition of IDD B3/State Single Service Recipients from Care Coordination to Providers

Effective 3/1/2016, CenterPoint will be discontinuing Care Coordination services for any person who is receiving only 1 paid service through Medicaid B3 and State funds.  Individuals with more than one paid service will continue to receive Care Coordination services from CenterPoint.

This change will impact 256 members and 20 network providers.  For those impacted, the duties of coordinating development of the person centered plan, requesting authorization, and monitoring progress toward goals will shift from the MCO Care Coordinator to the Provider Network Qualified Professional (QP).

To help providers prepare for this change, CenterPoint will be offering training at no cost on the skills and practices that Provider QPs will need to learn to effectively assume these duties.  Training topics to be addressed will include training on service definitions, IT training on submission of authorization requests, UM procedures and tools to help meet clinical documentation needed to maintain services, and guidance on use of planning tools and assessments.  

This training is not for all IDD Providers.  We ask that the 20 identified providers send 1 to 2 staff members.  Agency QPs must attend and it is highly recommended for at least one person in leadership attend as well. This training is scheduled for 2 full days.  Please choose from one of the two 2-day options in the drop down box.
