Planning for the Inevitable -- Helen Swanagon
Would you like to have answers to questions like: how can advance care planning ease your mind now about the death experience and the emotional burdens on your family? What are the benefits of communicating your end-of-life wishes? What are the pitfalls of neglecting this task? What are the considerations for choosing the right person to act on your behalf?
In this interactive workshop your instructor Helen Swanagon, RN, will provide the information you need about these and other questions. You will also learn about the Advance Health Care Directive and the POLST form (Physician Orders for Life Sustaining Treatment) which is rapidly replacing the DNR. These forms will be available at the class.
If you don't already have these documents in place, you will leave with much to think about doing. If you already do have these documents filled out, bring them along with you to the class and see if they reflect your current wishes. Each participant will leave with an Advance Directive and a POLST form which can easily be filled out.
Helen Swanagon, RN, worked in a local hospice agency for 10 years and now coordinates the Palliative Care Program at Marian Medical Center in Santa Maria. In this capacity, she provides advance care planning advice and counsels patients and their families who are coping with serious or life-threatening illness.
